Forms for Teaching Fellows


 

  • Application Materials for Teaching Fellows  Regarding the spring semester hiring process for teaching English and Comparative Literature classes.  The TF hiring process takes place once each academic year,  beginning at the beginning of February.
  • AP-2S Form (DOC): If this is the first time you will work at the University of North Carolina, please fill out this form and turn in to the Writing Program Office.  This form is mandatory before payroll forms can be completed and submitted in the online payroll process.
  • Direct Deposit (PDF)
  • Payroll Deduction Authorization for Graduate Students (PDF): In order to have your student fees deducted from your paycheck, complete this form and submit it to the Writing Program office by Monday, August 15, 2011.  Due each semester.
  • Waiver of Hours (PDF): If you will take less than 9 credits in the fall semester you need to fill out this form and turn it in to the Writing Program office before Monday, August 22, 2011. Please click on the above link, fill out the form on line, and print a copy for the WP office.
  • Graduate Student Health Insurance (DOC): Health Insurance forms may be turned into the Writing Program office beginning on July 1, 2011 and are due in to the Writing Program office by August 15, 2011.  Please note that you must be enrolled in classes for the fall semester before health insurance information can be submitted by the Writing Program office. The sooner you submit the form, the faster you will receive your health insurance card in the mail.   If you have any questions about your policy and coverage, please contact Hill, Chesson,and Associates.   • W-4 US Tax Withholding Form (PDF)
  • NC-4 Tax Withholding Form (PDF)
  • Video Authorization Form (DOC): This form should be used in any ENGL courses where students will be videoed for instructional purposes. Students must be at least 18 years old, must understand that they will not have the opportunity to rescind the contract (e.g., the problem might arise if a student becomes a celebrity), and must each sign individual contracts (everyone in the class signing the same contract will not suffice).
