3: Unclear: either the specified activity did not occur on the day observed, or a flawed attempt was made.

2: Activity was conspicuously absent or a deeply flawed attempt was made. Immediate attention is needed to correct such problems.

1: Serious and detrimental problems were observed; immediate attention and assistance is necessary.
